MPCS Registration 2025-2026

August Registration Art 2025

Registration will be available online from August 1st – August 19th. All information will be available here and a reminder will be sent via ParentSquare.

SAVE THE DATE: August 12th! Instrument Rental (Grades 3-8) will take place from 11:00 – 2:00 PM in Heffernan Hall & In-Person Registration will take place from 11:00 – 6:00 PM in the Eurythmy Room.

Every family must complete the applicable registration by August 19th.

Student Permissions & Waivers Form

Only one Parent/Caregiver can fill this out for each child. Must be logged into ParentSquare. 

Volunteer, Student & Family Partnership Agreements

EVERY Parent/Caregiver needs to read & sign this agreement. Must be logged into ParentSquare. 

Immunization Updates

IF APPLICABLE (ECE & 6th grade or exemptions), send updated immunizations or certifcate of exemption to Adele Sakich in our Health Office.

Required & Optional MPCS School Fees

Pay required fees & purchase your yearbook(s) via Infinite Campus or through the Front Office.

Free & Reduced Application

Qualified families will receive benefits that reduce or waive material fees, field trip costs and meal costs (if applicable). All information is kept confidential. Click on the INSTRUCTIONS box in the top right corner (after clicking the link above) to see if you qualify. Your MUST email your qualification letter to mealsbenefit@mountainphoenix.org to receive your benefits.

General Health Appraisal Form

IF APPLICABLE (ECE & New Students or Current Families with updated medical history), please send completed form(s) to Joy Wegs (ECE Students) & Adele Sakich (Grades Students)

Additional Medical Forms

IF APPLICABLE, please have the necessary forms filled out by your child’s physician and sent to Adele Sakich in our Health Office.

**District Online Registration**

EVERY New Family must complete this step by logging into Infinite Campus. You will receive a link from Jeffco. Reach out to Julie Ortiz if you have any questions.

Records Request Form

New Families transferring into Jeffco Public Schools need to complete the Records Request Form.

CURRENT FAMILIES

Annual Registration Update

EVERY Current Family must update contact information and ensure all parent/caregivers are added to the HOUSEHOLD by logging into Infinite Campus. 

EARLY CHILDHOOD FAMILIES

Vital Forms

All Early Childhood Families must fill out a Vital Record Form for each student.

Brightwheel Electronic Sign-In

Sign-in and sign-out is now paperless through the  Brightwheel app! ALL Early Childhood families (Preschool and Kindergarten) MUST accept the invitation that has been emailed to you. Please reach out to Joy Wegs if you have any questions.
