Mountain Phoenix follows the Colorado Department of Education and Early Childhood licensing requirements regarding immunization compliance.

More information about immunization compliance and rates, visit:

SB20-163 requires schools to actively and directly share school’s measles, mumps, and rubella (MMR) rate with parents via CDPHE’s annual parent letter. We have also included the immunization rates for the DTaP, HepB, IPV, and Varicella.

Medical and Non-Medical Exemptions

If you choose to exempt your child from any of the required vaccines, you will have to submit exemption certificates during August registration for the upcoming school year.
