Medical Forms & Requirements

General Health Appraisal

Please submit a general health appraisal for your student before the start of school if: 

 * they are an ECE student
 * they are a new MPCS student 
 * they have any recent changes to their medical history

Immunization Requirements

For your child to attend school, please submit current immunization records for your student if:

 * they are an ECE student
 * they are a new MPCS student
 * they are entering 6th grade
 * they have received any new immunizations in the last few months

If you choose to not follow the recommended immunization schedule, please submit a current immunization exemption certificate (see below).

Colorado law requires all students attending Colorado schools and licensed child cares to be vaccinated against certain diseases unless a medical or non-medical exemption is filed.  For more information about vaccine requirements, resources and exemptions, please visit the Colorado Department of Public Health and Environment’s School Immunization page.

Immunization Exemptions

All non-medical exemptions for immunizations need to be completed on a yearly basis and must be provided to the school in August. Non-medical exemptions must be completed after 06/30/2024 to be accepted. Each student will need their own exemption.

Option 1:  Complete a “Certificate of Non-Medical Exemption”, and have it signed by the student’s immunizing health care provider.

Option 2: Complete the Online Immunization Education Module. Download or print the Certificate of Non-Medical exemption provided at the end of the course, this does NOT have to be signed by a Health Care Provider. 

Medical exemptions do not expire and only need to be submitted to the school once. This will need to be signed by a student’s immunizing health care provider.

Medications & Health Plans

All health plans and medication forms need to be completed on a yearly basis, new forms must be signed and dated after 06/30/2024. All medications at school, including prescription, over the counter, herbal/homeopathic, and essential oils must have a doctors order and held in the health room. The only exception to this is for a life saving medication, epi pen and/or an inhalers can be carried on a student with a self-carry contract on file.

If your student will need any health related accommodations during the school day, a health plan can be created for them. These are handled by our district nurse; Wendy Moore, we*********@ch***************.org , 720-951-7492.

Please work with your student’s doctor to get these forms updated and completed before the start of the school year.

All forms can be uploaded to Infinite Campus or submitted to the health room in person or emailed.
